Lots of Nutrition in Tiny Packages: Itty-Bitty Grains and Seeds

This week I worked with tiny grains and seeds: millet and teff, amaranth and chia seeds. These can be challenging, and I had a few starts and stops along the way until I came up with a collection of recipes I was pleased with, including some delicious, moist savory pancakes made with beet greens and cooked amaranth. I continued working with chia seeds; this time I didn’t soak them, and added them to whole-grain muffins and pancakes. That’s how I used to use chia seeds way back when, before they disappeared for a few decades from the whole-foods scene. You can raise the nutritional content of your whole-grain pastries and breads just by adding 3 or 4 tablespoons; along with their considerable nutrients, the seeds contribute interesting texture like that of poppy seeds.

You Can’t Patent Mother Nature

In a rare unanimous decision, the US Supreme Court ruled that human genes cannot be patented:

The case, AMP v. Myriad Genetics, revolved around Utah corporation Myriad Genetics’ exclusive patents on the BRCA1 and BRCA2 genes, which, when mutated, lead to a very high risk of breast and ovarian cancer. Because Myriad was the first to identify the BRCA genes, it patented them, charged exorbitant prices for BRCA testing, and then aggressively prevented any other labs from offering the same test. In 2009, a coalition of plaintiffs including the ACLU, Breast Cancer Action and a number of scientific organizations, researchers and patients sued Myriad, claiming that it had no legal right to hold patents on the BRCA genes.

In a majority decision written by Clarence Thomas, the court affirmed the plaintiffs’ claim that because DNA is naturally occurring, it “lie(s) beyond the domain of patent protection.” In so deciding, the court effectively reversed decades worth of policy by the US Patent and Trade Office, which has granted thousands of gene patents, many of which should now be rendered invalid.

The Court refuted Myriad’s claim that because it had put a lot of time and money into locating the gene, that it therefore deserved a patent: “extensive effort alone is insufficient” to make something patent-worthy. Basically, just because you tried really hard doesn’t mean that you deserve a multi-billion dollar legal monopoly.

This is great news for women, men, doctors, scientists and the world in the fight against breast and ovarian cancer. In a New York Times article about the impact of the ruling, other research companies said they would begin offering genetic testing which would bring down the cost and availability of the test, as well as, other tests held by patents:

Some experts say that other genetic tests that are exclusively controlled by a patent holder include the test for spinal muscular atrophy and the test for an inherited form of deafness.

Dr. (Sherri) Bale of GeneDx said the deafness gene also caused a skin disease. Her company is allowed to test for mutations that cause the skin disease, but if it discovers a mutation for hearing loss, it cannot tell the doctor. Instead, a new blood sample has to be drawn and sent to Athena Diagnostics, which controls the testing for the deafness gene. Dr. Bale said the court’s decision should eliminate the need for that arrangement.

It is often said that patents cover 4,000 human genes, or about 20 percent of all human genes, meaning the decision could have a large impact.

Amy Goodman and Juan Gonzalez of Democracy Now, in a discussion of the ruling, were joined by Judge Robert Sweet, the senior federal judge for the Southern District of New York who originally invalidated Myriad Genetics’ patents; Lisbeth Ceriani, one of the plaintiffs in the ACLU lawsuit. In May 2008, she was diagnosed with an aggressive form of breast cancer; and Sandra Park, a senior attorney with the ACLU’s Women’s Rights Project and a lead counsel on the case.

“With the ruling today, we fully expect much better access and much better options for patients, as well as for scientists who want to look at different parts of the genome,” Park says. “They no longer now need to deal with patents on the thousands of genes on our genome when they’re engaging in their scientific work.”

Soups With Spinach, Five Ways

There’s so much beautiful spinach in the farmers’ markets now, and though it will soon give way to summer heat, it’s nice to know you can buy it every week and make something different with it each time. If you’re already into hot weather, you’ll appreciate this week’s cold yogurt soup with spinach and grains, which I’ve been polishing off for lunch every day this week. But even the hot soups will work on a warm day.

New Ways to Use ‘Ancient’ Grains

Some of these have fed people in far-flung parts of the world for a very long time but have been of little interest as a food on the American market. Grains like sorghum have been grown here but fed primarily to animals, whereas sorghum has been a staple for centuries in Africa and India. [..]

These are chewy, hearty grains. They’re good with vegetables and with beans. Barley is wonderful in soups, and all of the chewy grains lend themselves to salads and to being stirred into cold yogurt soups along with other vegetables and herbs.

Bulking Up Smoothies With Chia Seeds

When you soak the seeds in water, they expand and become gelatinous, a property that aids digestion and contributes to their low glycemic index. When I use the seeds in smoothies, dressings and juices, I scoop up a tablespoon of the gelatinous mixture of seeds suspended in water – which is the equivalent of a teaspoon of unsoaked chia seeds – and add it to the drink or dressing. It adds substance to a drink, and I felt incredibly well nourished by this week’s chia-enriched fruit smoothies. I made five of them, adding other soaked nuts, seeds or muesli along with the chia. They made energy-rich breakfasts, perfect food for a morning workout.

Focaccia is a flatbread, not unlike a very thick-crusted pizza. It’s an easy dough to put together, and it’s forgiving; if you don’t have time to go from start to finish in one session, you can chill the dough and come back to it later. I think it’s a great vehicle for all kinds of vegetables, just as pizza is. A square of focaccia topped with tomatoes or cauliflower makes a great lunch or snack, and it’s good lunchbox fare.

Another thing you can do with focaccia is split it laterally and fill it, to make a sandwich. I did that with a mix of goat cheese and spinach, which made a better filling inside a heated focaccia than a topping.

Sweet and Easy Vegetable Pickles

The pickled vegetables make great snacks and hors d’oeuvres. They look beautiful on a platter. They’re also good with a sandwich or with cottage cheese, a quick and easy way to make vegetables part of your lunch.

Note that this week we are not including nutritional information with the recipes. It isn’t possible to do the analyses accurately, because we don’t know how much of each vegetable you will eat, and each vegetable absorbs a slightly different amount of brine and ingredients in the brine. I weighed the brine before and 10 days after pickling each vegetable and found that not much of the brine had actually been absorbed. As for the yield and serving sizes, the recipes make 1 to 2 cups of pickled vegetables. The number of servings really depends on how you are serving them. They will serve more as a nibble than as a salad or side.

Dukkah: Nut and Spice Mixes for Seasoning and Snacking

I gave you a recipe for dukkah, a Middle Eastern nut and spice mix, a few weeks ago. In that recipe I sprinkled it over the ingredients in a rice bowl. I’d made much more dukkah than the recipe called for, and found myself snacking on it until I ran out. Now it’s my favorite snack. I sprinkle some into my hand or into a ramekin and eat it by the pinch. Dukkah has so many of the attributes of a snack food – crunch, a little bit of salt (as opposed to a lot of salt), spice. I realized that very little salt is required when the salt is combined with spices and ground or chopped nuts and seeds to give your palate that hit of snack-food pleasure. And it occurred to me that dukkah could also fit the bill as a low-sodium seasoning for all sorts of dishes.

Pastas of Spring

    The beautiful, sweet vegetables of spring ― artichokes and peas, favas and tender young asparagus, spring garlic and sweet spring onions ― come and go so quickly that I find myself impulse buying at the market and using them up in the simplest of dishes. They beg nothing more than pasta, and that’s a good thing because many of these vegetables are labor-intensive. It’s worth the time it takes to shell the peas, to free the heart of the artichoke from its leaves, to shell and skin favas. Then little more is required than a quick sauté or simmer with aromatics. You can always embellish, though, as I am doing this week with some recipes, with a pesto or, in the case of a baked orzo pastitsio with artichokes and peas, a béchamel.

Rice, No Bowl Needed

I’ve always been a big promoter of brown rice, and I was happily testing recipes last week for short-, medium- and long-grain varieties when I remembered that there had been reports in the past year about dangerous arsenic levels in rice, particularly in brown rice. I thought about shelving the recipe tests and choosing another subject for this week’s Recipes for Health, but then I decided to take a closer look at the reports to see if there was a way to make rice viable for health-conscious rice lovers.

The study and report, both by Consumer Reports, are disturbing. It is clear that the levels of inorganic arsenic in rice and rice products are high, and that we and especially children, babies and pregnant women should limit our intake of rice and rice products such as cereals, rice cakes and rice beverages. Rice cereal for babies should not be the go-to baby food that it has been for years.

The better news is that the extensive testing by the Consumers Union of many brands of rice and rice products shows that some products are considerably lower in arsenic than others.

Boston Marathon, How They Saved So Many Lives

Last week I wrote about the use of tourniquets in a pre-hospital emergency to save someone from bleeding to death. Tourniquets were used very effectively in Boston and were a key factor in getting so many to the hospital alive. At the finish line there were two medical tents with cots, blankets and IV fluids to treat dehydrated, hypothermic runners. Both were staffed with doctors and nurses. An emergency-room physician from Georgia, Dr. Allan Panter, was waiting at the finish line for his wife. He was just 10 yards a way from the first explosion. He assisted with victims and after went to the tents to assist. He described the events:

While there was some initial chaos in a medical tent near the finish line, and some screaming and moaning by victims, it was generally an orderly scene, Dr. Panter said. He assisted others in wheeling in a female victim who died, he said. He described 20 to 30 cots in the tent with IV bags that had been intended for dehydrated runners.

At least eight doctors and what seemed to be 20 or more nurses were stationed in the tent. A man with a microphone stood in the center of the tent to coordinate medical care. Arriving victims were assessed and categorized as 1 for critical, 2 for intermediate, 3 for “can wait” and “black tag” for anyone who appeared to be dead, Dr. Panter said. An emergency medical technician outside the tent coordinated ambulance service to hospitals.

“All in all, it was a pretty controlled environment,” said Dr. Panter, who has been an emergency-room physician for 30 years. “I’ve seen a lot worse. They were without question ready – not ready for those type of injuries, but they were prepared.”

There usually aren’t those provisions or medical staff on site and this still required the actions of bystanders to help control bleeding and move patients to the tents and ambulances. The night after the bombings on MSNBC’s “The Last Word,” host Lawrence O’Donnell spoke with Dr. Lyle Micheli, the head physician at the finish line and Massachusetts General Emergency Room Nurse Meghan McDonald about their experience:

But what happened in the hospitals was even more critical. It wasn’t like the ER’s were empty and waiting for these patients. As Nurse McDonald described in the interview Massachusetts General hospital had 90 patients being treated, waiting for admission or discharge when the explosions happened. The other four other ER’s that would receive the bulk of the casualties were in not much better shape. Luckily they all have similar disaster plans in place and have frequent drills to keep the staff prepared. Prepared they were. Of the initial 170 patients the five level one trauma centers received that day only one patient, who arrived in cardiac arrest, died. The other two fatalities were pronounced dead at the scene. That is a quite a feat and a testament to the training and skills of the doctors, nurses and other support staff. This article in the New York Times describes how Massachusetts General Hospital handled the disaster:

The first priority for those who were severely injured was to prevent them from dying, often from bleeding to death. Many had tourniquets on their legs when they arrived at the hospitals. But that was just a temporary measure to slow the bleeding. They needed immediate surgery to get their bleeding under control and prevent muscles and nerves from dying for lack of blood. [..]

That requires a vascular surgeon to repair the torn blood vessels and restore blood to legs and feet that may no longer have a blood supply. To do those repairs, surgeons often sew in part of a vein from the other leg, if it is uninjured, or from an arm. Or they use a synthetic tube.

Meanwhile, an orthopedic surgeon must stabilize a bone that might be flopping because it is fractured in several places. Surgeons do that with a temporary solution – they drill into the bone from outside the leg and attach pins that they screw into a metal bar also outside the leg.

Plastic surgeons clean the wound. In this case, blast victims had BBs or nails or debris embedded in their legs and feet. Everything the surgeons took out of the wounds was placed in plastic bags for the F.B.I., said Dr. Samuel J. Lin, a plastic surgeon at Beth Israel Deaconess Medical Center who helped care for blast victims.

“The crime scene extends to the hospital,” Dr. Lin said.

It’s definitely an art. It might appear chaotic to the casual beholder as everyone seems to be moving and talking at once. Each staff member has his or her job and is looking and listening so as not to miss details. Usually there is one coordinator, in situations like this there are some times more, as the ER is sectioned off into areas that depend on the patient’s status. Life threatening are first, then go back to treat and repair everything else. The decision to amputate a limb is not made lightly and is done most often to save a patient’s life. The other reason is that the bone, tissue and vascular damage is so severe there is no other option. The doctors in Boston had the luxury of having an immediate second opinion, it doesn’t always happen that way in combat zones or parts of the third world.

MSNBC’s Rachel Maddow saluted the thorough, professional and remarkably successful performance of the medical professionals who responded to the emergency injuries of the Boston Marathon Bombings and kept the public informed with honest straightforward briefings.

I routinely throw them into salads of all kinds, and finely chopped walnuts can go into everything from omelets to pungent Mediterranean nut-based sauces to soups, pasta dishes and of course desserts. I consider them a pantry staple and keep a bag of shelled walnuts in the freezer at all times. When unshelled walnuts are available at my farmers’ market I keep them on hand as well. I use them up so quickly that I probably don’t need to keep the shelled walnuts in the freezer, but that’s where I always keep my nuts, because the oils in nuts are volatile and they can become rancid if they are not kept in a cool environment. I toast walnuts occasionally, but most often I prefer the sweeter flavor of fresh untoasted walnuts.
